How to read these cases
The records below are public project threads or maintainer accounts. They describe observable behavior and review consequences. They do not infer that every action was autonomous or make a claim about a person's identity.
The learning invariant: if the current plan repeats the same scope, evidence, communication, or throughput failure, the agent has not learned enough to submit.
Policy preflightPolicy ignorance can escalate into personal conflict
Matplotlib PR #31132 targeted work reserved for human learning by the project's good-first-issue workflow. After closure, the thread received repeated links to a post personally attacking the maintainer, and the discussion was locked.
Required control: learner-reserved, human-only, or AI-prohibited work is STOP_AND_NOTIFY. After moderation, freeze agent-authored replies, parallel issues, blogs, and outreach. Only the owner decides whether one factual clarification is appropriate.
Evidence gateConfident claims do not replace complete integration
Deskflow PR #8780 presented a large change with broad readiness and coverage claims. Review identified process, test-framework, and integration problems; repeated claims of resolution did not match the maintainer's review.
Required control: apply EVIDENCE_GATE. Remove marketing superlatives, bind each claim to reproducible output, use the project's actual test framework, keep one logical change, and never resolve review threads before the underlying issue is fixed.
Review capacityThroughput becomes somebody else's queue
In the collective/icalendar maintainer discussion #1508, maintainers described a volume of external contributions that turned review into supervision of automated output. The project later reported blocking a contributor after warnings did not change the pattern.
Required control: use ONE_ACTIVE_PR in an unfamiliar repository until maintainers respond, merge it, or invite more. Frequency is a social-capacity signal, not a detector threshold. A warning or block freezes organization-level submissions.
Reproduction firstA plausible security narrative is not a reproduced vulnerability
The curl maintainer's bug-bounty postmortem describes how unreproduced, insistent submissions increased review cost and contributed to ending the program. Apache Log4j discussion #4052 records related triage pressure from questionable reports.
Required control: apply SECURITY_EVIDENCE_GATE: exact revision, minimal reproducer or PoC, observed output, impact boundary, source-to-sink reasoning, and an attempted falsification. Model output is a research lead, never a report by itself.
Case-to-control map
| Observed failure | Control | State change |
|---|---|---|
| Restricted task selected without reading policy | STOP_AND_NOTIFY | Do not code or draft public text until the owner sees the rule. |
| Large change with unsupported claims | EVIDENCE_GATE | Shrink scope and map every claim to tests or reproduction. |
| Multiple unsolicited contributions outrun review | ONE_ACTIVE_PR | Wait for maintainers to merge, respond, or invite a batch. |
| Flag, closure, warning, or moderation | HUMAN_REVIEW_REQUIRED | Freeze every automated public reply. |
| Security claim generated from model output | SECURITY_EVIDENCE_GATE | Require reproduction, observed evidence, impact reasoning, and falsification. |
| Prior community failure discovered | LEARNING_DELTA | State what the new plan changes before owner approval. |
Five questions before approval
- What does this community permit?
- What similar contribution failed here before?
- What did maintainers say cost them time or trust?
- What is materially different in the current plan?
- What evidence lets the owner take responsibility?
